Summary

This is a follow-up to #634, which cut Phase A item 3 and the Phase C result-table sentence from swarm/SKILL.md. It fixes the two references those cuts left behind.

• Phase A numbering. The source list read 1, 2, 4, 5, so that Phase B's "the step 4 model" kept pointing at the model step. Rendered Markdown renumbers the list 1 to 4, so on GitHub the model step shows as 3 while Phase B says 4. The list now reads 1 to 4, and Phase B says "the step 3 model". Source and rendered view now agree.
• Phase D's "the table". The Phase C sentence that introduced the result table is gone, so "the table" had nothing to refer to. Phase D now asks for "a result table".

Four lines change, in this one file.

Notes

• No other file refers to swarm's step numbers. poteto-mode/playbooks/autopilot-full.md mentions a "Step 4", but that's its own step.
• feat(pstack): support Claude Code as a harness #629 also edits the Phase A model step and keeps its number 4. Whichever PR merges second will need a one-line rebase.
• No version change.

Test plan

• git diff --check is clean.
• scripts/validate-plugins.mjs checks manifests only, and this PR changes none.

Follow-up doc fix in swarm/SKILL.md after Phase A item 3 was removed elsewhere: Phase A steps are renumbered 1–4 (was 1, 2, 4, 5) so source numbering matches rendered lists on GitHub.

Phase B now refers to “the step 3 model” instead of step 4, aligned with the model-pick step. Phase D asks for “a result table” instead of “the table”, since the Phase C intro to that table was removed.

Hey @mstephenholl, the renumbering reads right against #634's cuts. One mechanical thing before a maintainer can merge: pstack/skills/swarm/SKILL.md is mirrored into harness/pstack/claude-code/skills/swarm/SKILL.md by scripts/generate-pstack-harness-trees.mjs, and the validate workflow runs that script with --check, which fails when the source and the mirror differ. The workflow has not run on this PR yet (fork PRs wait for approval), so it is not visible here. Running node scripts/generate-pstack-harness-trees.mjs once and committing the regenerated file makes the check pass.
