**A hand-written trigger header is no longer merged.** In 1.x, `TriggerEvent` added events to a JSON value
already set in `Response.Headers`. In 2.0, set these headers only with `TriggerEvent`; a value set another way is not kept.

Other changes:

- `TriggerEvents(IReadOnlyDictionary<string, object>, HtmxTriggerTiming)` was removed. Call `TriggerEvent` once per event.
