Summary

Fixes #174.

• Refresh the bundled skill at the beginning of activation migration, independently of desktop MCP configuration, configured agents, and accepting onboarding.
• Use the existing shared installer and extension-host home paths; a fresh remote/container home always gets ~/.agents/skills/debug-live, plus the existing optional Copilot target.
• Install once per activation instead of once per configured agent, and refresh after extension updates.
• Route installation success, missing-bundle warnings, and copy errors through the DebugMCP logger.
• Leave desktop configuration path detection and agent configuration content unchanged; those no longer gate skill discovery.

Validation

• npm run compile and npm run lint passed.
• 34 focused tests passed, covering activation without agents/configuration, malformed configuration, no DebugMCP entry, repeated activation, fresh remote-home target selection, real file copying, update refresh, and existing agent migration.
• git diff --check passed.
• No live Dev Container was available; remote-home filesystem conditions were tested with temporary directories.

Existing PR check
